HCD is committed to demonstrating care and compassion for patients, family caregivers, health care professionals, and to providing a great work-life balance for our staff. Due to our continuing growth, we are now looking to add a Patient Care Coordinator to our Home Care Delivered team.

The Home Care Delivered Patient Care Coordinator ships medical supplies to existing customers in compliance with company and regulatory policies/directives. The Coordinator will effectively communicate with patients in a confident, knowledgeable, highly professional, and caring fashion. The ideal New Patient Services Coordinator candidate has a high school education/GED, one year experience in a relevant field of work, strong computer skills, and an exceptional telephone manner. The following is a representative list of the duties and responsibilities associated with this position:

-- Communicates with patients in a highly professional, caring and knowledgeable manner.
-- Maintains current knowledge of, and adheres to the Company’s product formularies.
-- Accurately and expediently faxes physician order requests to physician offices.
-- Maintains high level of product knowledge.
-- Provides patients with a comprehensive explanation of required documents and explains how to complete required documents, as necessary.
-- Consistently ensures order accuracy by reviewing Company standards, requirements and policies and by performing self quality checks.
-- In accordance with department policy, thoroughly notes patient accounts when action is required/taken and/or upon each completed or attempted contact with the patient.
-- Accesses systems and prioritizes workload based on department parameters.
-- Effectively gathers and articulates customer information over the telephone to a diverse group of people, i.e. varying ages and physical and mental abilities.
-- Meets and/or exceeds productivity expectations.
-- Efficiently uses available resources to accurately and efficiently process necessary transactions.
-- Researches issues thoroughly and effectively follows through to completion.
-- Expedites service in situations requiring urgent attention as determined by management.
-- Identifies opportunities for improvement and contributes to solutions to enhance workflow efficiency and enhance the customer’s experience.
-- Understands workflow processes of contiguous departments and uses knowledge to make effective processing decisions.
-- Works independently and/or collaboratively to answer inquiries and resolve issues within regulatory and Company requirements to ensure customer retention and satisfaction, escalating issues whenever appropriate.
-- Achieves or exceeds assigned work standards.
-- Performs all duties in full compliance with all federal and state statutes, regulations and policies, CHAP requirements and policies and HCD's Compliance Program and Standards, including but not limited to, maintaining strict confidentiality and privacy of patients' protected health information (as defined by HIPAA) in accordance with HIPAA regulations, cooperating with internal and external investigations, reviews and audits, engaging in good faith reporting of compliance concerns, and participating in annual and ad hoc compliance training.
 
 
REQUIREMENTS
  Ability to speak, write and read handwritten and printed words and numbers in Spanish & English utilizing good grammar, spelling, and proofreading skills required
  High school diploma/GED required
  One year experience in a related field (call center, medical product sales, or Home Health setting)
  Familiarity with medical insurances, including Medicare and Medicaid, highly preferred
  Ability to speak distinctly and communicate in a friendly, upbeat manner via telephone; professional, results oriented communication skills; and ability to maintain rapport while achieving results is required
  Strong computer and Internet skills
  Attention to detail, multi-tasking skills, and a get-it-done work ethic a must
